Main Purpose/Scope:

Reporting to the Manager or designated site RN, LPNs provide and oversee all services that support a home-like environment for residents.  They work closely and cooperatively with the Manager or designated site RN, and Home Care, oversee and work with Health Care Aides to report and complete the required paperwork, and perform tasks that require specialized training within the scope of their position.  LPNs shall work within their scope of practice, competencies, and professional conduct as defined by the Health Professions Act or governing professional organization (CLPNA).

Key Responsibilities and Duties:

  • Be familiar with and perform tasks as outlined in the Alberta Licensed Practical Nurses Competency Profile for Gerontology Nursing
  • Oversee, provide guidance, and work with Health Care Aides (HCAs) to ensure proper delivery of services as outlined in resident Care Plans
  • Perform assigned tasks as directed by Home Care and approved by the Manager or designated site RN
  • Provide direct care and support to residents which includes, but may not be limited to:
    • assisting residents with the activities of daily living such as bathing, dressing, grooming, ambulation, and toileting as per their individual care plans
    • providing personal care to residents as outlined in their individual care plans
    • providing resident support for housekeeping, meal preparation and service, and recreational activities
    • performing assigned tasks as provided by Home Care and approved by GAF Management
    • assisting with therapeutic regimes as required: range of motion exercises, medication assistance and reminders, simple wound care, respiratory therapy, urinary and peri-care, etc.
    • assisting with mobilization as required: bed transfers, lifting, turning, walking, and crutch walking
  • Provide a variety of therapeutic and recreational activities that promote and enhance the independence model   
  • Distribute medication following Foundation policies and procedures
  • Administer insulin when required (no other injections approved without direct authorization from the Manager)
  • Monitor and report changes in resident's health that may affect their personal and/or health care needs to the Manager or designated site RN and/or Home Care
  • Complete all necessary paperwork and reporting requirements in a clear, concise, and timely manner
  • Respond to emergency calls from residents and take appropriate action
  • Communicate issues or concerns to the Manager or designated site RN
  • Maintain a neat, clean, pleasant, home-like environment for the residents
  • Participate in resident care conferences as required
  • Other duties as required

Job Specifications:

Education/Training:

  • Licensed Practical Nursing (LPN) Diploma
  • WHMIS training

Experience:

  • Related nursing experience and providing personal care to seniors considered an asset

Skills/Knowledge:

  • Ability to communicate clearly and effectively in written and spoken English

  • Ability to analyze information and evaluate results to provide solutions and resolve problems

  • Ability to handle difficult and/or crisis situations tactfully and diplomatically

Requirements:

  • Follow a shift rotation schedule including days, evenings, nights, and weekends
  • Complete work-related training as required
  • Support and adhere to all current and future Foundation policies and procedures
  • Support and adhere to all federal and provincial legislation as it applies to the Foundation including, but not limited to, Alberta Housing Act, Freedom of Information and Protection of Privacy Act, Protection for Persons in Care Act, Occupational Health and Safety, and WHMIS
  • Uphold all applicable standards including, but not limited to, Supportive Living Accommodation Standards and Continuing Care Health Service Standards
  • Participate in regularly scheduled staff meetings
  • Ensure compliance with dress and grooming policy to project and maintain a professional image
  • Mobility to work in multi-storey building and physical stamina to lift and carry up to 10 kg.

Specific Requirements:

  • Current satisfactory Criminal Record Check
  • Annual proof of active registration with the College of Licensed Practical Nurses of Alberta (CLPNA)
